Gynaecomastia is the enlargement of male breast tissue. It may affect one or both breasts.

It is a very common problem, affecting more than half of normal adolescent boys going through puberty. When gynaecomastia starts in puberty, it usually goes away by itself over time.

Gynaecomastia that starts in adulthood affects about one third of men. Adult gynaecomastia has many causes including hormonal changes, genetic conditions, weight gain and the side effects of some prescription medicines. Using anabolic steroids or marijuana are also common causes of gynaecomastia.

If you are a man with enlarged breasts, you may feel self-conscious and it may be affecting your quality of life. You may avoid swimming or taking your shirt off in front of other people. You may make jokes about yourself, trying to hide your embarrassment and prevent other people from commenting first. It’s not a problem that men tend to discuss, even with their partner or family doctor.

Q: What are the treatment options for gynaecomastia?

Treatment for gynaecomastia may include surgical removal and/or liposuction, depending on the cause and the type of enlarged breast tissue you have.

Some men need both types of treatment, and these can be done at the same time.

Surgical removal (called ‘excision’) is used to remove lumps of glandular breast tissue.

Surgical removal is recommended when you need lumps of glandular breast tissue or excess skin removed.

You are usually given pain-relief and sedative tablets that you take by mouth before the procedure. If needed, intravenous sedation given by an anaesthetist can be used.

Dr Kerry will make small incisions to remove the excess tissue and skin. To help reduce scarring, the cuts are usually in line with natural skin creases or around the edges of the areola. After removing the excess tissue and skin, Dr Kerry will use small, fine stitches to close the incision/s and will apply a special dressing to the area. The surgery usually takes about one hour to complete.

Traditional or smart liposuction is used to remove excess fatty tissue in the breast area.  

Liposuction is recommended when you need excess fatty tissue in the breast area removed.

Depending on how much fatty tissue you need removed, Dr Kerry will recommend either traditional liposuction (to remove larger areas of fat) or smart liposuction, which is also called ‘laser lipolysis’ (to remove smaller amounts of fat).

If you are having traditional liposuction, you will be given intravenous sedation by an anaesthetist. If you are having smart lipo, you will usually just need pain-relief and sedative tablets that you take by mouth.

Both types of liposuction involve inserting a thin cannula (tube) in the area being treated. In traditional liposuction, the fat cells are removed by suction applied to the tube. In smart lipo, the fat cells are gently heated with a laser so they liquefy. The liquidised fat cells are then suctioned out and any remainder is then naturally eliminated by your body over the next few weeks.

Wearing compression garments after either type of liposuction will speed recovery, improve your results and keep swelling to a minimum.

Q: Can all cases of gynaecomastia be treated by Dr Kerry?

Sometimes, there is an underlying medical cause of gynaecomastia. If Dr Kerry thinks this may be the case, he will order medical tests. He will also review any medicines you are taking that may be contributing to the problem.

If gynaecomastia has been caused by use of anabolic steroids or marijuana, you will need to stop using these substances. If you continue using them, even if you have surgical treatment and/or liposuction, your gynaecomastia will return.

In complicated cases, where you may need a breast reconstruction, Dr Kerry will refer you to a plastic surgeon who specialises in this type of surgery.

Q: What can I expect after gynaecomastia treatment?

If you have had lump removal only, the area will be covered with a dressing and compression garment. You will be ready to be driven home soon after your procedure. Your stitches will be removed one week after surgery.

In the first days after lump removal, you will have some swelling, bruising and pain. You can manage these by using pain relief medicine prescribed by Dr Kerry. The pain is similar to the tightness you feel after exercise or manual work and usually lasts for four to seven days. Our staff will provide you with clear printed directions for your post-operative care. Bruising will usually fade after the first week, and the swelling will gradually reduce over several weeks.

You will need to avoid lifting and certain types of physical activity for up to two weeks after surgery. Dr Kerry will explain this to you at your initial consultation and following your surgery. If your job is very physical, you will also need up to one or two weeks off work.

If you have had liposuction, you will need to wear a compression garment across your chest to speed recovery, improve your results and keep swelling to a minimum.

Recovery from traditional liposuction is usually rapid. Most people are able to return to normal activities within a week — although very strenuous activity and lifting is discouraged for several weeks.

If you have had smart liposuction, your recovery will be even quicker and you will have very little swelling, bruising or discomfort.

Our staff will provide you with clear guidelines about your care and activity level after either type of liposuction.

If you have had both lump removal and either form of liposuction, our staff will give you printed aftercare information for both procedures.

Q: How soon will I see the results of gynaecomastia treatment?

How quickly you see results will depend on the specific type of treatment you have had.

If you have had lump removal only, the results will be visible as soon as your swelling has gone down.

If you have had traditional liposuction, you will have some swelling and, and so you won’t immediately see the results. It usually takes several months to see the final results.

If you have had smart liposuction, you will see the results of treatment after one to three months.

If you have had both lump removal and traditional liposuction, you may have some additional swelling, and so it may take longer for you to see the final results.

With all types of treatment for gynaecomastia, we will take photographs and measurements before your treatment and at each followup appointment. This way, you will be able to see an objective measure of your progress.
